Video One - What are Tethered Oral Tissues (TOTs)

The first video in Sara's series Understanding Tongue Ties with Village Lactation — "Beyond the Frenulum". What tethered oral tissues actually are, the difference between the lingual, labial and buccal frenulum, and why every one of us has them. A frenulum on its own is never a diagnosis: two babies can have frena that look almost identical and feed completely differently, which is why Sara assesses function rather than appearance.

How restricted tongue movement can affect both breastfeeding and bottle feeding. Sara talks through what parents commonly notice — long feeds, coming on and off the breast, clicking, a shallow latch, feeding very frequently, nipple pain or damage, wind and unsettled feeds — and explains why these signs are clues rather than answers. Bottle feeding is covered too, because the idea that tongue ties only affect breastfeeding is a myth.

A release creates the opportunity for new movement, but it does not teach a baby how to use it — like taking off a tight pair of shoes you have worn your whole life, it takes time to adjust. Sara covers what normal healing looks like (the white or yellow diamond under the tongue that surprises many parents), why progress is not linear, how oral motor support differs for newborns and older babies, what to look at if feeding does not improve, and the emotional side of the decision — one she has been through with both of her own boys.

Six of the most common tongue tie myths, taken apart one at a time: that finding a tie automatically means a procedure, that ties only matter for breastfeeding, that a posterior tie is always the hidden cause of feeding problems, that difficult feeding must be because of the tie, that a release immediately fixes feeding, and that what you read online is always accurate. The question that cuts through all of them is not "does my baby have a tongue tie" but "how is the tongue functioning".
